Abstract

|
목적: 본 연구는 국민건강영양조사 자료를 이용하여 아동 천식과 아토피피부염에 영향을 미치는 요인을 규명하기 위한 서술적 조사연구이다.
방법: 본 연구는 보건복지부, 질병관리본부가 공동으로 수행한 2011년 국민건강영양조사 제5기 2차년도(KNHANES V-2) 자료를 이용한 2차 분석으로 12세 미만 아동 1,170명을 대상으로 SPSS를 이용한 복합표본 분석을 하였으며 각 단위별 가중치를 적용하였다.
결과: 본 연구를 통해 아동 천식에는 3세대 가족구성, 주관적 건강상태에 대한 인식이 보통이하인 경우, 그리고 어머니의 천식이 영향요인으로 확인되었고 아토피 피부염은 주관적 건강상태에 대한 인식이 영향요인으로 확인되었다.
결론: 아동기 주요 문제가 되고 있는 천식과 아토피 피부염 모두 부모로부터 인식되는 주관적 건강상태와 관련이 있는 것으로 나타났는데 주관적 건강인식이 삶의 질을 반영하는 결과인 만큼 아동의 천식과 아토피 피부염은 질병으로써의 치료뿐만 아니라 가족과 아동을 모두 포함하는 포괄적인 건강증진에 초점을 두는 중재방안이 필요하다. 본 연구를 토대로 천식과 아토피 피부염 아동과 가족의 삶의 질 향상을 위한 간호중재 방안을 마련하는 연구의 계기가 마련되기를 기대한다.

Purpose: The purpose of this study was to investigate factors affecting childhood asthma and atopic dermatitis.
Methods: For this study, data for 1,170 children (1-11 years) from the Fifth Korean National Health and Nutrition Examination Survey (KNHANES V-2, 2011) were analyzed. First correlation matrices were computed to test the normality of every data set and then to be in accordance with the real demographic composition, data were added weight before being analyzed.
Results: The child with asthma of a three generation family (OR=3.91, 95% CI [1.33, 11.45], p=.013) compared with a two generation family showed higher asthma development, and maternal asthma (OR=9.71, 95% CI [2.66, 35.40], p=.001) showed higher asthma development in child. The only factor affecting atopic dermatitis was parental perceptions of child health: poor (OR=3.40, 95% CI [1.29, 8.98], p=.014).
Conclusion: These results suggest that childhood asthma and atopic dermatitis are both affected by parental perceptions of child health. Accordingly, management and support programs for children who have asthma and/or atopic dermatitis and their families should be comprehensive and also give attention to any other health problems because health perception represents quality of life.
